First Annual Wilmington Jaycees Scavenger Photo Hunt

March 27, 2010. Sign-in at 10:30 a.m., Hunt starts at 11 a.m.

Each team member must read the waiver, fill the form out in total and submit payment through Paypal (at the bottom) to participate in the Wilmington Jaycees Scavenger Photo Hunt.

Waiver, Indemnification, and Release of Liability

As a condition for participating in the in the Wilmington Jaycees Scavenger Hunt, I attest to the following statements:

1. I understand that entering the Wilmington Jaycees Scavenger Hunt is a potentially hazardous activity.

2. I certify that I am medically fit and able to participate in the Wilmington Jaycees Scavenger Hunt .

3. I certify that I will obey all traffic, civil and criminal laws at all times.

4. I assume all risks associated with competing in the Wilmington Jaycees Scavenger Hunt, including but not limited to physical injuries, damage to my personal property, contact with other participants, the effects of the weather, including high heat and/or humidity, traffic and the conditions of the road, all such risks being known and appreciated by me.

5. I grant permission to all of the foregoing to use any photographs, motion pictures, recordings or any other record of this event for any legitimate purpose including but not limited to promoting, advertising and marketing purposes. Any and all photographs, motion pictures, recordings or other records of the event and are and will become the sole property of the Wilmington Jaycees.

6. I understand that all entries are final with no refunds.

7. Waiver & Release of all claims & assumption of risk I recognize and acknowledge that there are certain risks of physical injury to participants in the race/activities, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minor child/ward may have (or accrue to me or my child/ward) as a result of participating in these activities against the Wilmington Jaycees, including its officials, agents, volunteers, co- sponsors, and employees (hereafter collective “the Jaycees”). I do hereby fully release and forever discharge the Jaycees from any and all claims for injuries, damages, or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ ward and arising out of, connected with, or in any way associated with the race, program or activities.

8. I agree to indemnify and defend the Jaycees for any and all claims that may arise from the Scavenger Hunt as a result of my participation in the Scavenger Hunt.

9. I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. When registering online, I understand that my online signature shall substitute for and have the same legal effect as an original form signature. PARTICIPATION WILL BE DENIED, if the Signature of an adult participant or parent/guardian and date are not signed at the time and place of the race.

I do hereby fully release and forever discharge the Jaycees from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with the Scavenger Hunt.

I have read and fully understand the above important information, warning of risk, assumption of risk, indemnification and waiver and release of all claims.
